In this unit, you were introduced to high-consequence, low-probability (HCLP) events. This assignment will allow you to explore one of these types of events in more detail.
The investigations of the Macondo well blowout and explosion/Deepwater Horizon offshore platform disaster of 2010 and the Fukushima-Daiichi tsunami-caused nuclear meltdown catastrophe of 2011 have concluded that both accidents could have been prevented. For this assignment, choose one of the two referenced HCLP events, and address the following items.
Summarize the selected disaster.
Identify the contributing factors to the failures.
Discuss ways in which they could have been prevented.
Your paper should be at least two pages in length, not counting the title and reference pages. A minimum of two sources must be used, one of which may include the textbook. Consider reviewing the resources given in the suggested unit resources section of this unit, and be sure to include concepts you learned in this unit in your response. Follow APA Style when creating citations and references for this assignment. APA formatting, however, is not necessary.
